I authorize the persons and organizations named in this application to provide inSite Housing, Hospitality and Health Services Inc. with any and all information and records concerning my previous employments, education, character, and qualifications in compliance with the employment legislation of Community Care and Assisted Living Act and its regulations, The Income Tax Act, The Employment Standards Act, and the Canadian Provincial and federal Labour Codes. I understand that any misrepresentation, falsification, or material omission of information on this application may result in failure to receive and offer, or if I am hired, in my dismissal from employment, regardless of when such falsification or omission is discovered.
